Training Class Registration Form (Continuing)

INSTRUCTIONS

This form may only be used for dogs that have previously trained with Jumping Chollas. If you are entering the Foundation and Relationship class please use the registration form for new students.

Direct training questions to the Training Director at trainingdirector@jumpingchollas.com or 602-439-2784. If you submit your class application through this form, your application will automatically be sent to the Training Director.

If you choose to send your application through the mail send your applications for classes other than puppy classes to:

The Junior Handler Class is an 18 week program that's offered twice a year - in the fall and spring. The next class begins on September 25, 2007.
