Training Class Registration Form (Continuing)
INSTRUCTIONS
This form may only be used for dogs that have previously trained with Jumping Chollas. If you are entering the Foundation and Relationship class please use the registration form for new students.
Direct training questions to the Training Director at trainingdirector@jumpingchollas.com or 602-439-2784. If you submit your class application through this form, your application will automatically be sent to the Training Director.

Junior Handler Classes
The Junior Handler Class is an 18 week session held on
Thursday evenings at 6:15. There are 2 sessions a year, in the spring

Note: Please enter this form for each dog. The waiver of liability form only needs to be signed once if you have enrolled more than one dog in classes .
